The New York State Veterans' Home at St.
Albans is a healthcare facility owned and operated by the New York State Department
of Health for Veterans' and their dependents. We are located in Jamaica NY, about
13 miles from New York City. We are a 250-bed facility and have been serving the
Veterans' and their dependents of the New York City region since 1993.
The New York State Veterans Home at St.

Administration Primary and Extended Care Center in the St. Albans Section of Queens

The Admissions Department is responsible
for the overseeing of the admissions process. They are available to give personal
tours of the facility, and meet with you to discuss any questions you may have regarding
admissions.

Upon admission, the unit social worker
will greet you and your loved one at the door and will assist you in getting acquainted
with the unit, staff and facility. Social workers are available to meet with you Monday
through Friday, 8:00 AM until 4:00PM, and are available evenings or weekends when
necessary.
The social workers acts as liaison, resident advocate, or moderator for you or
your loved one regarding individual issues or other departments to ensure that your
personal, social and emotional needs are being met. Your social worker continually
monitors for changes in mood, behavior and cognition. When appropriate the social worker
will meet with you and/or your loved one to devise interventions to address your needs
which could include but is not limited to; adjustment to long term care, grief counseling,
support groups, and discharge planning where appropriate. The Social Services department
consists of three social workers including a registered and certified social worker.
Medicaid is a program for New Yorker's
who can not afford to pay for medical care. You may be covered by Medicaid if:
- You have high medical bills
- You receive Supplemental Security Income (SSI)
- You meet certain income, resource, age, or disability requirements.

The New York State Veterans' Home is a

We, the staff of the New York
State Veterans' Home at St. Albans have been given a great opportunity to serve those
who have served us in the armed forces of the United States of America. The Human
Resources Management Office works with each Department to pick the very best candidate

Our campus is a
special community providing needed services, and companionship for veterans
and dependents. It's physically attractive grounds and accommodations, combined
with a sincere atmosphere of caring, makes life at the Veterans' Home pleasant,
enjoyable, secure and comfortable.
Our main floor serves as the hub for all our central services. These services
include Gift Shop, Library, Clinic, Rehabilitation Services, Veteran Lounge,
Café, Meditation Room, Barber/Beauty Shop, Multi-purpose Activity Room
and a Climate Controlled Atrium.
The
Business Office at The New York State Veterans' Home at St. Albans takes care
of billing, purchasing, resident accounts, Medicaid/Medicare, and resident mail.
Residents may establish an individual
interest bearing account through the Veterans' Home Residents Accounts office. You may
deposit funds into this account to pay for personal items or services such as the
Barber/Beauty Shop, Gift Shop, Cable TV, or Telephone. A statement is automatically
provided to you or your designated representative quarterly or upon request. Banking
hours are Monday, Wednesday and Friday 2:00 PM to 3:00 PM, or upon request.

Mail is delivered to the Veterans' Home
business Office Monday through Friday. Resident's personal mail is sorted and delivered to
their room by the staff on the unit. Residents may choose to have the Business Office
staff open and manage their business mail, such as insurance, physician, or hospital
bills. We will only do this with your express written permission, which can be revoked
at any time. You may also wish to handle these items yourself. Fell free to visit us for

We provide skilled nursing care to
Veterans' and their dependants. We also offer a range of medical, vision, and
dental services. Our activities department provides recreational programs for
our residents to enjoy. The Food and Nutritional Services department is
responsible for the healthy diet of each resident. Teamwork is promoted through
each department to ensure a maximum quality of life for our residents.
Each of the Home's
residential units houses people in single or double rooms with a bathroom.
Each room also has a private wardrobe, individually controlled heat and
air conditioning, TV and telephone hook ups. For health and safety reasons,
smoking is not permitted in resident rooms or most other areas of the
facility. designated smoking areas (indoors and outdoors) are provided.
Two units have been designed as specialized units. Our 15-bed sub-acute
unit is available for residents who require higher levels of skilled nursing
and medical care. A 35-bed Dementia unit is designed to assist residents with
Alzheimer's disease and other related dementias. An added security system and
unique recreational programming is in place to preserve the quality of those
residents living on this unit.
Residents with special needs receive all of the general long-term care services
as well as the special services they require.
The New York State Veterans'
Home Medical Services include the operation and supervision of the onsite
Clinic, Admissions, Pharmacy, Medical Records, and Discharge Planning.
The clinic consists of the attending physicians (including the Medical Director),
Dentist, Podiatrist, Ophthalmologist, Physician's Assistan, and the Support
Staff.
The Nursing Department
at the New York State Veterans' Home provides skilled nursing care on 7 units
ranging in size from 15 beds to 40 beds per unit including the Sub Acute Unit
and the Dementia Unit.
The Department consists of over 200 employees including:
- Registered Nurses
- Licensed Practical Nurses
- Certified Nursing Assistants
- Secretaries, Ward Clerks
Our Nursing Department employees range from brand new graduates of Certified
Nursing Assistant Schools to seasoned employees with over 20 years of
experience. We provide educational opportunities for our employees through
onsite sessions and by having the flexibility to attend classes elsewhere.
The Food and
Nutritional Services Department at the New York State Veterans' Home at St.
Albans provides nutritional meals to all the residents of the Home.
Dining areas on each unit afford residents, a smaller home-like eating area
while under the care of our Nursing staff or if they choose a quiet meal can be
provided. The residents benefit from our state of the art "Heat On
Demand" tray system that assures hot, nutritious meals for each resident.
All residents receive not only the meal plan prescribed for them by our Medical
Staff, but snacks throughout the day. Every effort is made to meet the personal
wisher of our residents at meals and at snack time. The facility's resident
menu is a four week cycle menu that affords each resident a minimum of two choices
per meal but can be up to, six choices per meal. This is in addition to any
personal food preference a resident may have. Residents are encouraged to participate
in the facility's menu planning at the monthly meeting of the Food Committee,
which is open to all residents of the Home.
The Medical Nutritional Care of our residents in handled by a full time team of
two Registered Dieticians and three Diet Technicians. The clinical staff is
available to discuss meal preferences with residents and family members. The
team works closely with the Medical Staff, the Speech and Language Pathologist
and the Nursing Staff to provide a dietary regime that maintains or improves
the nutritional health of our residents. Residents requiring tube feedings and
mechanically altered diets as well as those able to tolerate a regular diet
are all closely monitored by the clinical nutrition staff.
The New York State
Veterans' Home boasts of a fully equipped Rehabilitation Department ready to
meet your short and long term needs. The professional staff of therapists work
closely as a team to help people achieve their maximum potential.
The Physical Therapists develop a plan to improve the person's movement and
function. New equipment recently purchased at the Veterans' Home consists of
Kinetron, Access Weight Trainer, Ultrasound, Electric Muscle Stimulation, Fluidotherapy,
and Whirlpool treatments. The Physical Therapists frequently work with individuals
who have had strokes, fractures, joint replacement, and arthritis, to name a few.
Occupational Therapists work closely with people to increase their activities of
daily living and independent function. Activities may range from dressing and self
care to food preparation. Treatments for these skills bridge the gap between nursing
home placement and successful return to the community.
The Speech Therapist works closely to improve communication skills and dysphagia.
All therapists are part of an interdisciplinary team that helps the resident
achieve his or her goals.
